How to Register!

1.BOTH parent & student must sign the TRAINING AGREEMENT!
2. HAVE Certificate of Completion from online provider.
3. Copy of permit.
4. Our Policy Agreement signed & dated by BOTH parent & student.
5. Fill out Emergency Medical Form.
6. Payment of check or cash.
7. All registrations are accepted either by mail or by dropping them off at
the business in the mailbox by our business door. Please be sure
to leave a convenient time to call and schedule!
8. All forms MUST be complete and payment included
BEFORE we will schedule Behind the Wheel!

Additional Information
1. 8 hours of private one-on-one instruction with a Certified Instructor
2. Drop off & pick up @ Debbie's Driving Academy
3. Scheduled on a first-come, first-served basis
4. Must have permit with you doing behind the wheel training
